Wegovy now comes in two forms that share the same active ingredient, semaglutide. One form is a once-weekly injection, while the other is a once-daily tablet that you swallow in the morning.

Both are prescription medicines used with a reduced-calorie diet and more physical activity for chronic weight management in adults with obesity, or adults with overweight plus at least one weight-related condition. They are also approved to lower the risk of major cardiovascular events such as heart attack or stroke in adults with known heart disease and either obesity or overweight.

The injection has an additional FDA-approved indication for adults with noncirrhotic metabolic dysfunction-associated steatohepatitis, also called MASH, with moderate to advanced fibrosis.

But which is better? This article explains how the Wegovy pill vs. injection compares, covering dosing, effectiveness, side effects, safety, costs, and what to discuss with your clinician.

Approved Indications

The Wegovy pill and the Wegovy injection are approved to be used alongside a reduced-calorie diet and increased physical activity. They are prescribed for long-term weight loss and weight maintenance in adults with obesity, or in adults who are overweight and have at least one weight-related health condition.

Both forms are also approved to lower the risk of major cardiovascular events, such as heart attack and stroke, in adults with established cardiovascular disease who have obesity or are overweight.

However, the injectable form of Wegovy has additional approved uses that do not apply to the pill. These include:

    • Use for pediatric patients aged 12 years and older with obesity.
    • Treatment of noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH), formerly known asnonalcoholic steatohepatitis (NASH), with moderate toadvanced liver fibrosis in adults.

Which is better for weight loss?

Both forms are highly effective compared with placebo when used as directed, and both have produced double-digit average weight loss in clinical trials.

In the STEP 1 trial, adults with overweight or obesity without diabetes received once-weekly Wegovy injection 2.4 mg or placebo, along with lifestyle intervention.

After 68 weeks, the average weight loss was about 15.3 kg (33.7 lb) with Wegovy injection compared with about 2.6 kg (5.7 lb) with placebo. A greater proportion of participants in the Wegovy group achieved weight loss of at least 5% (86.4% vs. 31.5%), 10% (69.1% vs. 12%), and 15% (50.5% vs. 4.9%).

Approval of oral Wegovy was supported by the OASIS program, including the OASIS 4 trial. In this study, adults with overweight or obesity were assigned to Wegovy tablets 25 mg taken once daily or a placebo, alongside lifestyle intervention.

The average weight loss at week 64 was about 15 kg (33 lb) with the Wegovy tablet compared with about 2.7 kg (6 lb) with the placebo. More participants taking the tablet achieved weight loss of at least 5% (76% vs. 31%), 10% (60% vs. 14%), and 15% (47% vs. 6%).

However, we can’t compare those numbers head-to-head, since the trials differ in design, timepoints, and populations.

That said, pharmacokinetic studies suggest that once-daily administration of the Wegovy 25 mg tablet is predicted to be comparable to the Wegovy 2.4 mg once-weekly injection, although with higher variability for tablets.

Dosing schedules and titration

Dose escalation is used for both forms to reduce gastrointestinal side effects. Here’s how they differ:

How they are taken

The Wegovy injection is given once weekly as a subcutaneous shot, usually in the abdomen, thigh, or upper arm. Meanwhile, the Wegovy pill is taken once daily by mouth, following strict timing rules.

The injection is flexible. It can be taken with or without food, at any time of day. The pill must be taken first thing in the morning, on an empty stomach, with a small amount of water, and no food, drinks, or other medications for at least 30 minutes afterward.

Switching between the Wegovy pill and injection

When switching from the injection to the tablets, adults who are taking Wegovy 2.4 mg injections for weight reduction or cardiovascular risk reduction may transition to Wegovy 25 mg tablets. The tablets should be started one week after the last 2.4 mg injection, at a dose of 25 mg taken once daily.

When switching from the tablets to the injection, patients may transition from Wegovy 25 mg tablets to Wegovy injections. The 2.4 mg injection should be started the day after the last 25 mg tablet, and then continued once weekly.

For some patients with type 2 diabetes who need additional weight reduction while on tablets, the label notes switching to 1.7 mg injection and following the injection escalation schedule.

Missed doses

Missed dose rules differ between the tablet and injection.

If you miss a Wegovy tablet dose, the prescribing information says to skip the missed dose and take the next dose the following day. Do not double up to make up for the missed day.

If you miss one injection dose and the next scheduled dose is more than 48 hours away, take the missed dose as soon as possible. If the next scheduled dose is less than 48 hours away, skip the missed dose and take the next dose on your regular day.

If two or more consecutive weekly injection doses are missed, the label advises either resuming as scheduled or restarting and following the escalation schedule, which can reduce gastrointestinal side effects when reinitiating.

Which causes more side effects?

Because the active ingredient is the same, the side effect profile is similar. The most common side effects are gastrointestinal.

Commonly reported reactions include:

    • nausea
    • vomiting
    • diarrhea
    • constipation
    • abdominal pain

Many people see these effects most strongly during dose escalation, and they often improve as the body adjusts.

Discontinuation due to adverse reactions occurred in both injection and tablet trials. If tolerability is an issue at the 25 mg tablet maintenance dose, the prescribing information suggests considering a switch to 1.7 mg injection once weekly.

Storage and handling differences

The Wegovy injection requires refrigeration, temperature monitoring, and more careful handling, while the pill is easier to manage, especially for travel or daily use.

Wegovy injection (pens)

  • Refrigeration is required before use. Unused pens must be stored in a refrigerator at 36°F to 46°F (2°C to 8°C).
  • Do not freeze. Pens that have been frozen should not be used.
  • Limited room-temperature storage. Once removed from the refrigerator, pens can be kept at room temperature up to 86°F (30°C) for a limited time, as specified in the product labeling.
  • Light protection. Pens should be kept in their original carton to protect them from light.
  • Careful handling. Pens should not be shaken or exposed to excessive heat.

Wegovy pill (tablets)

  • Room-temperature storage. Tablets are stored at 68°F to 77°F (20°C to 25°C), with short-term excursions allowed.
  • No refrigeration needed. This makes tablets easier to store and travel with.
  • Moisture protection. Tablets should be kept in their original packaging and protected from moisture.
  • Simpler handling. Tablets do not require special handling beyond standard medication storage.

Cost and access considerations

The cost of Wegovy injection is around $1,349.02 per package, which adds up to over $16,000 per year. Each package contains four pens for weekly injections. Similarly, the list price for Wegovy tablets is also $1,349.02 for a 30-tablet bottle for all tablet strengths, including 1.5 mg, 4 mg, 9 mg, and 25 mg.

However, Novo Nordisk also promotes a much lower self-pay option for people paying out of pocket. Under this program, monthly pricing starts at $149 for the 1.5 mg tablet and increases to $299 per month for the 9 mg and 25 mg doses. The 4 mg tablet is offered at $149 per month for new patients through April 15, 2026, after which the price increases to $199 per month. On the other hand, the injections are offered at $349 per month, across all doses.

Outside of these manufacturer-supported programs, the monthly cost of the Wegovy pill is essentially the same as the injection.
